Job Description:

The Registered Nurse Care Coordinator works with patients identified as at-risk due to substance use disorders and related medical or behavioral health needs. The RN collaborates with an interdisciplinary care team to identify and close gaps in preventive care, chronic disease management, and regulatory requirements. This role supports the delivery of high-quality, efficient, and cost-effective healthcare services by providing patient education, promoting self-management, supporting evidence-based care plans, and coordinating transitions of care.

  • BSN or ADN degree from an accredited school or college of nursing

  • Current RN license in the state of North Dakota, Minnesota, and/or Wisconsin upon hire. Ability to obtain RN license in all 3 states within 3 months of hire.
